(I) In case of application by a company or partnership, it must be signed by the person(s) with the legal qualifications, and the company's seal must be affixed. Attach a copy of the certificate of incorporation, a certificate indicating the person(s) binding the company/partnership, and a certificate from the Registrar of Companies and Official Receiver confirming no bankruptcy has been declared. The original certificates must be issued by the Registrar.

(II) In the case of section 5 of the application, a copy of the high school diploma and a Certificate of Criminal Record must be attached.

(III) Cheques must be issued to the Council of Commercial Agents.
